We accept most insurance and offer cash packages
Scan the list of insurance providers on this page to verify that yours is listed. If it is not, you may contact us to see if we can add them. We also offer two cash packages as an option for those who do not have or do not wish to bill these services through their insurance provider.
We maintain state-of-the-art electronic medical records
A key to making health care work better for you is our use of Electronic Medical Records (EMR). HeartCare has been keeping patient data electronically for more than a decade. Few physicians have taken the time to convert their paper files, but doing so increases the ease, accuracy, and confidentiality of patient data. Having accurate data in an easy to access format helps your health professionals make better and more timely treatment decisions.
We provide forms for you to fill out in advance
For your convenience, New Patient Registration and History Forms are available for download in English and Spanish. You can print them and fill them out before your visit and fax them to HeartCare or bring them with you to your initial consultation.

Based on your history and blood profile, HeartCare will determine and
conduct the specific tests you need. Treatment
HeartCare will create a customized treatment program for you. Follow-up Visits
HeartCare will monitor and support your progress.
Forms
After you make an appointment, you may print and complete the patient intake forms. Fax them to HeartCare (323) 483-0735 or bring them with you to your first visit. If you fax them, call to verify we received them.
